Annual Leave Application

Please use the form to apply for annual leave.

Please ensure you have received full confirmation of annual leave before booking your own holiday. If we are unable to accommodate your request, an alternative will be provided. Notice period should be a minimum of TWO WEEKS for any annual leave booked.

Please allow for weekends and bank holidays when calculating total days off.

Mileage Claim Form

Please use the form to submit your mileage claim. If you have more than six mileage claims, please submit additional claim forms (simply refresh the page after submitting to renew the form).

You will be sent a copy of the form to the email address provided; please check the details and notify us of any errors as soon as possible.

Please note: Standard working rota mileage is pre-calculated. This form is for miles outside of standard rota, e.g. shopping, day trips out with clients. Mileage is paid for travel between client’s homes only, for welfare, monitoring and day opportunities.
